Social Services

Huntington Park Care Center addresses all social, and psychosocial needs / concerns of our residents. Our Social Service staff will coordinate with our Geriatric Counselor and / or our Geriatric Psychiatrist when issues arise that go beyond our scope of practice.

The Social Services department is available Monday through Saturday 8-5 to assist with admissions, discharges, and social service related tasks.  Similarly, we act as advocates to ensure compliance with Resident Rights.  At Huntington Park, we are “committed to a standard of excellence in providing quality care.”  We would be more than happy to arrange a tour at your convenience or to evaluate a potential resident for placement. Inquiry Information

Huntington Park Care Center is a skilled nursing facility (SNF) with 111 beds.  We are Medicare and Medicaid certified.  We offer skilled, rehabilitative, respite, hospice, and long-term care.  Our setting offers semi-private and private rooms.  Our facility is designed all on one level.  We offer three different levels of care at our facility (Light to moderate care, maximum care, and a special care unit for our residents with Alzheimer’s and other types of dementia.)  Our daily rates are dependent upon the level of care required. Rates are available upon request.

Admission Requirements 

We require that all residents obtain a current (within the last 30 days) History and Physical by their attending physician, indicating required level of care.  Medical information from a hospital, nursing home stay, or from an assisted living facility can also be helpful in evaluation of the potential resident.  Please have the proper professional fax these materials to 402-339-9603 if you are interested in HPCC. 

Our Admission Team will then evaluate the medical information of the potential resident for appropriateness of placement based upon physical and psychosocial needs.
